Services

Assisted Living

Tenants in Assisted Living manage their own lifestyle, and are encouraged to participate and become active members of our community. Attendants are available 24 hours a day to assist with personal care, homemaking services, and other activities associated with daily living. PHARA enables tenants to live independently.

  • Mobility/Ambulation/Transferring

  • Personal Grooming/Hygiene

  • Bathing/Washing/Toileting

  • Rising/Retiring/Dressing/Undressing

  • Meal Preparation

Scheduled services are available according to the individual’s Service Plan.

Assisted Living is offered in designated units within buildings operated by PHARA known as Westwinds Apartments, Westwinds Heights, and Westwinds Village in North Bay.

The program provides personal support services to adults in the North Bay and surrounding communities. Attendants provide assistance associated with daily living needs. Services are available at pre-scheduled periods to assist with personal care needs, homemaking services, and other activities associated with daily living needs. Services are available from 6 am to midnight. 

  • Personal Grooming/Hygiene

  • Bathing/Washing

  • Rising/Retiring

  • Toileting

  • Dressing/Undressing

  • Mobility/Ambulation/Transferring

  • Meal Preparation

  • Light Housekeeping/Laundry

Attendant Care Outreach also includes the Assisted Living for High Risk Seniors Program, which includes access to 24 hour support, and is by Home and Community Care referral only.

Post Stroke Transitional Care Program

The goals of the program are to maximize stroke recovery and provide multiple avenues for successful community reintegration. Offering individuals a comprehensive continuum of services, including education, socialization, recreation, assessment and therapy. As well as connecting individuals to needed community health and social services.

This program is in partnership with North Bay Regional Health Center.

Assistive Devices Program

We provide a free of charge service to individuals in the community who are in need of assistive devices. Members of the community are able to loan out an assisted device from PHARA on a 90 day period. Our Assistive Devices program* is stocked with assisted devices through donations from people in our communities. We have access to a variety or equipment including:

  • Rollator

  • Walker

  • Shower Chair

  • Cane

  • Manual Wheelchair

  • Crutches

  • Versa Frame

  • Commode

  • Bed Assist Rail
